The minimum structure that's actually useful

A working forecast needs, at minimum: an opening bank balance, expected receipts by week (grouped by customer or category, not lumped into one line), expected payments by week (payroll, rent, suppliers, tax and VAT payments as separate lines, not buried in "other"), and a rolling closing balance that carries forward. Thirteen weeks is a common horizon — long enough to see a real problem coming, short enough to stay accurate.

Where forecasts go wrong

The most common failure is treating invoiced revenue as received cash — a forecast built on invoice dates instead of realistic payment dates (accounting for your actual average collection period, not the payment terms on the invoice) will consistently overstate near-term cash. The second is forgetting irregular but predictable outflows: VAT payments, annual licence renewals, and bonus or gratuity provisions tend to get missed because they don't recur monthly.
